RELEASE FORMS

All participants for the Open Water Diver course, Scuba Skills Update, continuing education courses, and pool uses must fill out the SSI Scuba Waiver & Medical Form. 

This form is valid for one year from the date of signing. If you have completed this form within the past year, you do not need to complete another form.  However, if your medical condition has changed since the original signing, you may be required to complete a new form. 

NOTE: If there are any "YES" answers on the medical form, participant must get a physician sign-off on the printable Medical Form before entering the water.
